If your child will be taking prescription medication with them to DC, they will need a signed form from the prescribing physician. Different medications require a different form and each medication requires it's own form. I have linked all of them below along with a link to the HCSD list of forms. If you are not sure which one you need, please ask your physician.
THESE WILL BE COLLECTED IN THE FALL WHEN YOU COMPLETE YOUR CHILD'S EMERGENCY FORM. Please do not send them to school until the fall. However, you may want to get the forms signed at your child's next doctor visit because some offices will charge and can take up to 2 weeks to get the forms back to you.
If your child has a form on file with the school nurse - you DO NOT NEED another form for me. I will get it from the school nurse. This generally applies to anyone who takes medication at school or leaves emergency meds at school (Epi, inhaler, etc.).
